Manage pain management and symptom control for terminally ill patients per the plan of care, interdisciplinary team, and attending physician. Document assessments, interventions, goals, and outcomes while coordinating total patient and family care. Collaborate on the clinical component of the plan of care, ensuring continuity between facilities and providers. Supervise Hospice Aides and Licensed Practical Nurses, facilitating service delivery in long-term care settings. Provide continuing education on hospice concepts to families and participate in on-call rotations for after-hours, weekend, and holiday coverage.
