Hospice Registered Nurse (RN) Float per diem

As a Hospice Registered Nurse (RN) on the HopeHealth Hospice & Palliative Care team in Massachusetts, you will perform initial and ongoing assessments of patients and their needs, implement and adjust individualized plans of care, educate patients and families about the disease process and self-care, initiate nursing procedures, coordinate with the interdisciplinary team, and communicate changes in patient needs to the physician. The role supports continuity of care, discharge planning, and collaboration with family/caregivers to address physical, functional, psychosocial, and environmental aspects of care.
