Job Summary
Hospice Registered Nurse (RN) on the Home B team for HopeHealth Hospice & Palliative Care. Provide initial and ongoing assessment of needs related to terminal diagnosis, implement and revise individualized plans of care, educate patients/families/caregivers on disease process and self-care, initiate appropriate nursing procedures, coordinate all patient/family services with the interdisciplinary team, and communicate changes in patient needs to the physician. Per diem position offering flexible scheduling and rotation across Cranston/Providence/East Providence/Warwick service areas; delivers compassionate, autonomous end-of-life care in home settings with opportunities for growth and education within a not-for-profit hospice provider.
Required Qualifications
- RN required
- BSN preferred
- 1-3 years of related experience
- Hospice care and/or related experience preferred
