Hospice Registered Nurse (RN) Case Manager

Job Summary
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, utilizing the nursing process to write and revise care plans based on evolving status. Administer prescribed medications and treatments, counsel patients and caregivers on nursing and related needs, and teach self-care techniques including diet and symptom management. Coordinate care by communicating with physicians, community health personnel, and interdisciplinary teams while ensuring equipment and supplies are available. Manage a caseload of 12-15 patients across facility and home-based settings, supervise ancillary personnel as assigned, and participate in on-call duties and quality improvement activities. Maintain accurate clinical documentation and pursue continuing education to upgrade professional skills.
Required Qualifications
- A passion for hospice care and the ability to build relationships with both patients and their family members
- The ability to work efficiently with little direct supervision
- A valid driver's license and an insured automobile that is in good working order
- 1-2 years of recent acute care experience in an institutional setting (required)
- Graduation from an accredited school of nursing
- Understands hospice philosophy, secure with issues of death/dying
- Comfortable providing specialized care to the terminally ill
- Displays empathy for the needs of the ill, injured, frail and impaired
- Provides a calm demeanor when in a patient's home
- Monday – Friday; 8a-5p with some on-call and weekend rotation
- Mix of facility and home-based hospice care
- Managing an average caseload of 12-15 patients

Desired Qualifications
- 1-2 years of recent home care experience (preferred)
