Job Summary
Plan, organize, and direct hospice care by completing comprehensive assessments of patients and families to determine specific needs. Utilize the nursing process to write and initiate care plans, regularly re-evaluate status, and revise plans as patient conditions change. Administer prescribed medications, initiate preventive and rehabilitative procedures, and counsel families on nursing and related needs. Coordinate care with physicians, community health persons, and interdisciplinary groups while teaching self-care techniques and facilitating activities of daily living. Supervise ancillary personnel, participate in quality improvement teams, and fulfill assigned case responsibilities. Maintain accurate clinical documentation and engage in on-call duties and weekend rotations as defined by policy.
Required Qualifications
- A passion for hospice care and the ability to build relationships with both patients and their family members
- The ability to work efficiently with little direct supervision
- A valid driver's license and an insured automobile that is in good working order
- 1-2 years of recent acute care experience in an institutional setting (required)
- Graduation from an accredited school of nursing
- Understands hospice philosophy, secure with issues of death/dying
- Comfortable providing specialized care to the terminally ill
- Displays empathy for the needs of the ill, injured, frail and impaired
- Provides a calm demeanor when in a patient's home
- Monday – Friday; 8a-5p with some on-call and weekend rotation
- Mix of facility and home-based hospice care
- Managing an average caseload of 12-15 patients
Desired Qualifications
- 1-2 years of recent home care experience (preferred)
