Hospice Registered Nurse (RN) Case Manager

Job Summary
Conduct nursing assessments and coordinate patient/caregiver care, developing and updating plans of care to ensure uninterrupted delivery. Facilitate patient and caregiver independence while providing education on protocols, standing orders, and guidelines. Communicate effectively with the interdisciplinary group, patients, and external healthcare providers to maintain continuity and manage complex issues. Maintain accurate clinical documentation and apply infection control principles. Perform patient room cleaning and facility upkeep as required. Requires current active RN license, CPR certification, and minimum one year of related experience.
Required Qualifications
- A Diploma or associate degree in nursing
- Bachelor's Degree
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN)
- The Nurse Licensure Compact (NLC)
- Current CPR
- Minimum 1-year related experience for inpatient units (IPU)
- 2 years for other nursing positions
- Experience in direct care of patients with life limiting illnesses
- Related experience in hospice and palliative care
- Related experience in family medicine
- Related experience in internal medicine
- Related experience in geriatrics
- Related experience in oncology
- Related experience in home health care
- Related experience in medical surgical
- Computer proficiency
Desired Qualifications
- Certified Hospice & Palliative Nurse (CHPN)
