Job Summary
Hospice Registered Nurse (RN) Case Manager delivering patient and caregiver care, coordinating and delivering nursing care, conducting assessments, developing/updating care plans to ensure continuity of care, facilitating patient/caregiver independence, providing patient/family education, communicating with the IDG and other health professionals, applying infection-control practices, and maintaining accurate clinical documentation; requires active RN license (multistate/licensure compact) and CPR, with 1 year inpatient experience (2 years for other nursing roles); CHPN preferred; diploma/associate degree in nursing required; located in Greensboro, NC.
Required Qualifications
- A Diploma or associate degree in nursing is required
- A current, active, and unrestricted multistate license as a Registered Nurse (RN) is required
- The Nurse Licensure Compact (NLC) allows a multistate license in home state to practice in other compact states
- Current CPR required; CHPN preferred
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions; hospice/palliative care experience preferred
- Maintains current professional licensure and CPR certification; CEU requirements
- Computer proficiency required
