Hospice Registered Nurse (RN) Case Manager

Job Summary
Hospice RN Case Manager delivers coordinated patient and caregiver nursing care, conducts assessments, develops and updates care plans to ensure goals are achieved, educates patients and families, facilitates communication with the interdisciplinary group and other health professionals, adheres to infection control and universal precautions, maintains thorough clinical documentation, and pursues ongoing professional development while maintaining licensure and certifications; the role supports hospice and palliative care for patients across settings with a focus on compassionate, patient- and family-centered care.
Required Qualifications
- A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required
- Current CPR required; CHPN preferred
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions; experience in direct care of patients with life-limiting illnesses
- Computer proficiency is required
