Hospice Registered Nurse (RN) Case Manager

Job Summary
Conduct assessments for home and nursing home patients, including initial and ongoing evaluations, while collaborating with physicians to manage and regularly evaluate patient plans of care. Initiate preventive and rehabilitative nursing procedures, make necessary community service referrals, and provide ongoing training to patients and family members. Prepare point of care clinical documentation, coordinate service changes with the physician and staff, and determine service scope based on acuity. This full-time, 40-hour position includes rotating weekend commitments every fourth weekend. Required Qualifications
- Graduate of an accredited nursing school
- RN required
- Licensed as a Registered Nurse in Massachusetts and Rhode Island preferred
- Minimum of 1-3 years of clinical experience
- Excellent assessment, clinical and communication skills
