Hospice Registered Nurse (RN) Case Manager

Hospice RN Case Manager provides clinical oversight, care coordination, and education to patients and families in home-based hospice care across RI communities (Cranston, Providence, Pawtucket, East Providence, Warwick). Responsibilities include initial and ongoing patient assessments, collaborating with physicians on plans of care, evaluating and updating the plan to meet changing patient needs, providing ongoing training and education to patients and families, initiating appropriate nursing procedures, documenting care, coordinating services, and determining scope and frequency of services based on acuity. Requires graduation from an accredited nursing program and licensure as a Registered Nurse in RI (and MA preferred).
