Job Summary
Conduct skilled nursing visits in patients' homes and facilities, assessing symptoms, pain, and comfort needs. Develop, implement, and revise the plan of care with the interdisciplinary group and attending physician. Administer medications and provide expert pain and symptom management for patients at end of life. Educate and emotionally support patients and families throughout the hospice journey. Document every visit accurately and on time in the electronic health record, meeting visit and regulatory requirements. This per-visit role offers flexible PRN scheduling across New Hampshire with premium pay and mileage reimbursement for field travel.
Required Qualifications
- Active New Hampshire RN license in good standing (compact/multistate accepted)
- Current AHA BLS/CPR certification
- Valid NH driver's license
- Reliable, insured vehicle for patient visits
- Strong assessment skills
- End-of-life symptom management skills
- EHR documentation skills

- One year of RN experience
