Hospice Registered Nurse - PRN

Administer nursing care and skills to homebound patients while establishing treatment plans according to the Plan of Care. Monitor and report changes in patient symptoms or behavior, and manage multi-disciplinary care to promote continuity through appropriate admissions, transfers, and discharges. Communicate with the collaborating Interdisciplinary team regarding patient care, educate families on health maintenance, and provide emotional support. Document visits promptly in the Homecare Homebase electronic charting system. This flexible PRN role covers Winchester and surrounding areas with pay ranging from $32 to $45 per hour, including mileage reimbursement and a company-paid tablet. OASIS and HCHB experience is preferred.
