Conduct comprehensive start-of-care assessments, draft care plans, reconcile medications, and ensure accurate documentation for hospice patients. Coordinate care with families, provide health instructions per the assessment and plan, and submit proper reports to the Case Manager. Requires an active RN license with at least one year of experience, strong critical thinking skills, and reliable transportation for travel. Required Qualifications
- An active RN license in the state in which practicing
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
