Job Summary
Hospice Registered Nurse / RN Case Manager delivering compassionate, patient- and family-centered care in a home-based hospice setting. Build trusting relationships with patients and families, educate caregivers on care needs, collaborate with the care team to provide comfort and dignity, and enable patients to spend quality time with loved ones. Requires an active RN license in the work state, two years of RN experience (hospice experience preferred), a valid driver’s license, and current CPR/BCLS certification. Working conditions include part-time scheduling with flexible opportunities across Compassus’ continuum of home-based services; benefits and professional development support are provided.
