Job Summary

Provide skilled end-of-life care to patients and families while serving as a trusted mentor, educator, and clinical resource for nurses throughout their onboarding and professional development. Manage a patient caseload, coordinating individualized care from admission through ongoing visits while building meaningful relationships with patients, families, physicians, and the interdisciplinary care team. Guide new clinicians through orientation, provide ongoing coaching and remedial education when needed, and help students and staff build the confidence and competencies required to deliver outstanding hospice care. Work Monday through Friday, 8:00 am to 5:00 pm, with daily travel within the Greater Jackson service area to homes, assisted living facilities, or nursing homes.

Required Qualifications

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum 2 years of experience as a nurse in hospice or home health environment
  • Reliable transportation
  • Valid driver's license
  • Valid automobile insurance coverage
  • Nursing skills as defined by nursing standards
  • Ability to handle multiple priorities, documentation requirements and deadlines
  • Skill in nursing practice
  • Ability to cope with family emotional stress
  • Tolerance of individual lifestyles
  • Excellent observation skills
  • Analytical skills
