Hospice Registered Nurse On Call (RN Weekend days)

Job Summary
Assess, plan, implement, and evaluate patient care while managing pain control and symptom management for hospice patients. Coordinate with attending physicians, the medical director, and interdisciplinary team members to determine eligibility, develop care plans, and report condition changes. Administer medications, perform comprehensive death notes when appropriate, and counsel families on end-of-life needs. Maintain accurate point-of-service documentation and ensure equipment arrangements align with agency policy and Medicare regulations. Requires weekend on-call availability, local travel, and a State Certified RN license with hospice or home health experience.
Required Qualifications
- Associate's Degree in Nursing
- State Certified RN license
- Experience in either Hospice or home health care
- Must have excellent documentation skills
- Ability to work independently
- Strong communication skills
- Valid driver's license
- Reliable transportation
- Must be available to travel locally within assigned territory
