Job Summary
Assess and address the comprehensive needs of terminally ill patients and their families, initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care. Deliver skilled nursing care as outlined in individualized care plans while traveling daily to patient locations and facilities as assigned. Collaborate with physicians, social workers, chaplains, and nursing assistants to develop and adjust care plans, educating caregivers on techniques and advocating for patient wishes. Serve as a liaison between patients, families, and healthcare providers to ensure clear communication. Requires a valid New Jersey or Multi-State RN license with two years of acute-care experience.
Required Qualifications
- Valid New Jersey or Multi-State Registered Nurse (RN) license
- Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units
- Strong clinical practice
- Customer service
- Excellent communication abilities
- Capacity to work independently in a fast-paced environment
- Travel daily to patient locations and facilities as assigned
