Job Summary
Provide nursing care and documentation that meets reimbursement guidelines; coordinate physical care of patients with family, patient, physician, and hospice team; communicate with patients/families and agency personnel; attend and participate in IDT meetings and quality review; maintain compliance with nursing practice acts, agency policies, and reimbursement guidelines; this role requires RN licensure, nursing education from an approved program, and willingness to travel as needed.
Required Qualifications
- Current RN licensure in state of practice
- Successful completion of a nursing education from an approved school of nursing
- Two years of clinical nursing experience
- At least one year of hospice nursing experience preferred
- Knowledge of home health and hospice care program
- CPR certification
- 12 hours of continuing education annually
- Ability to travel based on agency need and weekend/evening/holiday call assignments
