Job Summary
Coordinate physical care of patients with families, physicians, and the hospice team while applying nursing theories to meet reimbursement guidelines. Document all care activities and communicate with patients and families at all levels. Attend interdisciplinary team meetings and quality reviews as scheduled. Maintain current RN licensure, complete annual continuing education, and hold CPR certification. Accept travel assignments and weekend/evening call rotations based on agency needs.
Required Qualifications
- Current RN licensure in state of practice
- Successful completion of a nursing education from an approved school of nursing
- Two years of clinical nursing experience
- One year of hospice nursing experience
- Knowledge of the framework, function, and organization of a home health and hospice care program
- Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines
- 12 hours of continuing education annually
- CPR certification
- Accept travel based on agency need
- Accept weekend/evening and holiday call assignments
- Background checks/drug-free workplace
