Job Summary
Coordinate physical care of patients and families facing terminal illness while applying nursing theories and documenting care to meet reimbursement guidelines. Communicate with patients, physicians, and all agency personnel, and participate in IDT meetings and quality reviews as scheduled. Manage weekend on-call rotations and travel assignments based on agency needs across Durham, Hillsborough, Orange, and Person counties. Requires current RN licensure, two years of clinical experience, and one year of hospice experience preferred.
Required Qualifications
- Current RN licensure in state of practice
- Successful completion of a nursing education from an approved school of nursing
- Two years of clinical nursing experience
- Knowledge of the framework, function, and organization of a home health and hospice care program
- Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines
- 12 hours of continuing education annually
- CPR certification
- Accept travel based on agency need
- Accept weekend/evening and holiday call assignments
- Background checks/drug-free workplace
Desired Qualifications
- At least one year of hospice nursing experience
