Job Summary
HOSPICE REGISTERED NURSE (RN) needed for Per Diem roles at Liberty Hospice. Responsibilities include applying nursing theories, documenting care to meet reimbursement guidelines, coordinating physical care with family, patient, physician, and Hospice team; communicating with patients/families and agency personnel; attending IDT meetings and participating in quality reviews. Requires current RN licensure, nursing education from an approved program, two years of clinical nursing experience with a preference for hospice experience, knowledge of home health/hospice care, familiarity with nursing practice act and reimbursement guidelines, 12 hours of CE annually, CPR certification, and ability to travel with potential weekend/evening/holiday call assignments.
Required Qualifications
- Current RN licensure in state of practice
- successful completion of a nursing education from an approved school of nursing
- two years of clinical nursing experience
- at least one year of hospice nursing experience preferred
- Knowledge of home health and hospice care program and nurse’s role in patient care
- Working knowledge of nursing practice act, policies and procedures, and reimbursement guidelines
- 12 hours of continuing education annually
- CPR certification
- Accept travel based on agency need as well as weekend/evening and holiday call assignments
