Job Summary
Plan, coordinate, and provide hospice care by conducting comprehensive assessments, obtaining medical histories, and performing physical examinations. Develop and implement initial plans of care while consulting with physicians and the medical director regarding condition changes. Evaluate patient, family, and home situations to determine necessary health teaching, dietary counseling, and available assistance. Submit weekly visit schedules, complete clinical notes, and supervise hospice aides per agency policy. Participate in case conferences, QAPI meetings, and staff development sessions while maintaining current knowledge of drug therapy and regulations. Perform on-call duties including holidays and fill in for other nurses as needed.
Required Qualifications
- Graduate from an accredited School of Nursing
- Licensed in NJ as a Registered Nurse by New Jersey State Board of Nursing
- One year of RN experience
- Excellent assessment skills
- Problem-solving skills
- Skillful in organization
- Knowledge of case management processes
- Licensed driver with an insured automobile in good working order
- Ability to communicate well, both verbally and in writing
- No exceptions to RN license
- Ability to administer pharmaceuticals
- Ability to perform medication reconciliation
- Ability to perform any nursing procedures or special treatments needed within the scope of nursing practice for state
