Job Summary
Deliver compassionate, patient-centered end-of-life care to patients in their homes in the Cheswick, PA area, collaborating with an interdisciplinary team focused on comfort and dignity. Perform comprehensive patient assessments, manage symptoms and pain, and develop individualized care plans while educating families through the end-of-life journey. Think critically to solve real-time problems and ensure thorough, compliant EMR documentation. Travel to assigned patient locations within your territory, spending approximately 20% of your day driving, supported by a dedicated field education team and leadership committed to your growth.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- Two years of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successfully complete a drug screen
- Be legally authorized to work in the United States
