Job Summary
Triage, coordinate, and administer hospice care for patients, families, and caregivers by utilizing hospice services and resources. Manage the plan of care from referral through discharge, completing initial nursing assessments and physical histories to determine needs. Implement interventions that promote optimal outcomes while maintaining dignity, and administer medications as prescribed by the physician. Collaborate with the interdisciplinary team to provide teaching on pain and symptom management, communicate with medical directors and attending physicians regarding patient needs, and participate in team meetings. Maintain accurate clinical notes, document measurable outcomes, and complete discharges including pronouncement visits. Provide a safe environment, keep supervisors informed of status changes, and contribute to the agency's Quality Improvement Program. This part-time evening shift role offers weekly pay and access to Pioneer Valley Hospice's extensive post-acute health care network across Massachusetts.
Required Qualifications
- Minimum of one (1) to two (2) years of experience in health care
- An associate's degree in nursing
- Current Massachusetts licensure: Registered Nurse
- Possesses and maintains current CPR Certification
- Valid driver's license
Desired Qualifications
- Experience in a hospice, or home health preferred
- Experience in acute care in an institutional setting preferred
- Understand hospice philosophy and issues of death/dying
- A bachelor's degree in nursing preferred
- Hospice and Palliative Care Nurse Certification preferred
