Job Summary
Provide skilled nursing care to hospice patients in their homes, conducting assessments, monitoring symptoms, and administering comfort-focused medications. Serve as the key point of contact for patients, families, and the interdisciplinary care team while delivering education and emotional support. Complete timely EMR documentation and participate in scheduled on-call responsibilities. This role offers full-time 40-hour coverage in the East Side of Cleveland with a $5,000 sign-on bonus, AI-assisted documentation tools, and PSLF eligibility.
Required Qualifications
- Active RN license (Ohio)
- current BLS certification
- Reliable vehicle
- valid driver's license
- ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 1 year of clinical nursing experience
Desired Qualifications
- Prior hospice experience
- Experience with end-of-life care and comfort-focused care delivery
- Familiarity with HCHB or similar home health/hospice EMR
- Strong communication and family support skills
