Job Summary
As a Hospice Registered Nurse / RN Case Manager, provide patient-centered hospice and palliative care in home settings. Build trusting, compassionate relationships with patients and their families; empower caregivers through education on care needs; collaborate with the care team to deliver comfort, peace, and dignity while supporting quality time with loved ones. Responsibilities include coordinating among disciplines to manage symptoms, advocate for patients, and support families throughout the hospice journey. Skills emphasized include strong clinical RN experience, hospice familiarity, effective communication, empathy, and a collaborative approach to care delivery.
Required Qualifications
- RN license in the state you work
- Two years of experience as a registered nurse (hospice experience preferred)
- Current driver’s license
- Current CPR and BCLS certification
