Job Summary
Oversee hospice/home care clinical operations as Hospice Registered Nurse Clinical Director, coordinating patient care and supervising clinical personnel. Responsibilities include reviewing case referrals and patient information, assigning appropriate hospice personnel, coordinating with the Medical Director on eligibility, guiding clinicians to improve quality of home care services, maintaining EMR workflow, assisting in establishing goals and plans of care, facilitating case conferences, participating in quality assessment and performance improvement teams, contributing to staff recruitment/orientation, and helping develop organizational policies and training programs. Requires RN licensure, BSN preferred, and relevant hospice/home care supervisory experience; strong communication, nursing judgment, and adherence to professional standards.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- BSN degree preferred or a minimum of one year experience as a professional nurse within the last three years
- Two years of management or supervisory experience in hospice/home care setting
- Complies with accepted professional standards and practice
- CPR certification
- Valid driver's license with automobile insured as required
- HCHB and PointCare experience preferred
