Hospice Registered Nurse Case Manger

Hospice Registered Nurse Case Manager (RNCM) to manage symptom control and plans of care for patients in a hospice setting. Responsibilities include symptom management in collaboration with the attending physician and hospice Medical Director, evaluating and individualizing the plan of care, ensuring continuity of care with admissions/transfers/discharges, educating and advocating for patients and families, comprehensive clinical documentation per agency protocol and Medicare/Federal guidelines, and supervising total care provided by nurse aides and LPNs. Requires RN license in the employing state and a valid driver's license; experience in acute care and hospice is preferred.
