Job Summary
Hospice RN - Case Manager coordinates and delivers home-based nursing care, develops written care plans for each patient on caseload, conducts ongoing interdisciplinary assessments, provides skilled nursing procedures and patient/family education, communicates with physicians, and participates in case conferences. Approximately 75% of charting is performed in the patient’s home. Supervises home health aides, LPNs/LVNs, and CNAs; participates in on-call and holiday rotations; ensures documentation and coordination of discharge planning and care plan updates.
Required Qualifications
- Must have graduated from an approved school of nursing
- Must be licensed in the State of Wisconsin
