Job Summary
Hospice - Registered Nurse Case Manager to coordinate and provide home-based hospice nursing care. Responsibilities include evaluating patient needs, developing and documenting individualized care plans with an interprofessional team and family/caregivers, ensuring continuity of care across the continuum, advocating for patient-centered care, and timely communication. Role operates Monday–Friday 8:00am–4:30pm with weekend and on-call rotations; includes sign-on bonus up to $15,000, relocation available, mileage reimbursement, and coverage across St. Louis regions. Requires registration with the Family Care Safety Registry and a safety-sensitive status. Preference for Bachelor's degree; pathway for RN Career Ladder advancement available.
Required Qualifications
- Nursing Diploma/Associate's
- RN license
- Valid Driver's License
- 2-5 years preferred experience (2-5 years listed as preferred)
- Bachelor's Degree preferred (Nursing/Home Health)
