Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs, then initiate and regularly revise care plans based on evolving status. Administer prescribed medications and treatments while counseling patients and caregivers on self-care techniques, pain management, and health instructions. Coordinate patient care as a case manager by communicating with physicians, community health resources, and interdisciplinary teams to ensure continuity. Maintain accurate clinical notes, report abuse or neglect to authorities, and participate in on-call duties and quality improvement activities.
Required Qualifications
- Graduate of an accredited school of nursing
- Registered Nurse, with a License in good standing to practice
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Baccalaureate degree in nursing
- Minimum of two (2) years' experience
- At least one of which is in the area of public health, home care, or hospice nursing
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
