Hospice Registered Nurse Case Manager (RN)

Provide care and services for a group of patients by visiting them in private homes, Skilled Nursing Facilities, and assisted living facilities. Contribute to the interdisciplinary team by performing psych-social assessments, interventions, and coordinating with nurses, social workers, chaplains, volunteers, and aide leadership. Manage visits, documentation, and meetings through phone, email, and written communication while prioritizing urgent needs. Maintain independent day-to-day operations with a valid driver's license to travel across the service area.
