Hospice Registered Nurse Case Manager, RN

Job Summary
Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs, including physical assessments and history of current and previous illnesses. Coordinate patient care for assigned caseloads by writing and initiating plans of care, regularly re-evaluating patient and family needs, and revising plans as status changes. Administer medications and treatments per physician orders, initiate preventive and rehabilitative procedures, and counsel families on nursing and related needs. Provide health care instructions, assist with activities of daily living, and facilitate efforts toward self-sufficiency or comfort care. Complete weekly visit minimums established by the Office Administrator, manage documentation, and respond to urgent needs by prioritizing multiple concerns. Requires current RN licensure, valid driver's license, and ability to travel across the service area.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system
Desired Qualifications
- Experience in nursing, with emphasis on community health education/experience
