Job Summary
Conduct initial assessments of patients and families to determine hospice care needs, then regularly re-evaluate nursing requirements and utilize assessment data to refine care plans. Collaborate with the healthcare team, patients, and families to direct continuous, well-coordinated work while counseling end-of-life needs and initiating preventative and palliative procedures. Communicate with physicians regarding patient conditions and supervise hospice aides. Work M-F 8AM-5:30PM with a valid driver's license and reliable transportation; travel is required.
Required Qualifications
- Associate's Degree in Nursing
- State Certified RN license
- 2 years' experience as a professional nurse
- valid Driver's license
- reliable transportation
- willingness to travel
- Strong verbal and written communication skills
