Hospice Registered Nurse Case Manager (RN)

RN Case Manager position in Norman, OK within a hospice setting. Responsible for completing initial patient assessments within 48 hours of hospice election, collaborating with the attending physician and interdisciplinary team to develop, coordinate, and initiate individualized plans of care, conducting ongoing assessments and updating care plans based on patient condition, addressing physical, psychological, social, and spiritual needs, maintaining regular communication with physicians and the care team to ensure continuity of care, ensuring accurate documentation to meet regulatory requirements, providing leadership by instructing caregivers and volunteers and supervising LPNs and hospice aides, practicing infection control, assisting with discharge planning, and participating in professional development, community programs, and agency initiatives.
