Job Summary
Hospice Registered Nurse / RN Case Manager (PRN) provides end-of-life care coordination, builds trusting relationships with patients and families, educates caregivers on at-home care, and collaborates with the care team to deliver comfort and dignity. Role responsibilities include enabling patients to spend quality time with loved ones, empowering caregivers through education, and working with the care team to manage symptoms and support overall well-being. Requires RN license in the state of work, at least two years of RN experience (hospice experience preferred), a valid driver’s license, and current CPR/BCLS certification. Based in San Antonio, TX with part-time scheduling.
