Hospice Registered Nurse Case Manager (New Braunfels/Seguin)

Job Summary
Conduct initial and regular physical assessments of patients and families to determine home care needs and nursing diagnoses. Initiate and revise care plans that establish goals, incorporating therapeutic, preventive, and rehabilitative actions while including patients and families in the planning process. Administer medications and treatments as prescribed, counsel families on meeting nursing needs, and provide health care instructions. Coordinate patient care as a Case Manager when assigned, prepare clinical notes for physicians, and ensure equipment and supplies are available. Participate in on-call duties and supervise home health aides every two weeks. Requires current RN licensure, CPR certification, and a licensed driver's license.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Self-directed and able to work with minimal supervision
- Excellent observation skills
- Problem solving skills
- Verbal and written communication skills
- Nursing skills per competency checklist
- Ability to organize and prioritize workload independently
- Prolonged or considerable walking or standing
- Ability to lift, position, or transfer patients
- Ability to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, or crouching
- Visual acuity and hearing to perform required nursing skills

- Certify that throughout my association with EH I will comply with the terms of the Code of Conduct and Compliance Policies
Desired Qualifications
- Minimum of two years' experience, at least one of which is in the area of public health or home care nursing
