Job Summary
Provide hospice RN case management to enable patients to spend quality time with loved ones. Build trusting relationships with patients and families, empower caregivers through education on care, and collaborate with the care team to deliver comfort, peace, and dignity in home-based hospice care. Requires RN license in the state of work, two years of RN experience (hospice experience preferred), valid driver’s license, and current CPR/BCLS certification.
