Job Summary
Hospice Registered Nurse / RN Case Manager providing in-home hospice care and coordinating patient and family care. Build trusting relationships with patients and families, empower caregivers through education on care, and collaborate with the interdisciplinary care team to deliver comfort, peace, and dignity. This role involves enabling patients to spend quality time with loved ones, coordinating care plans, and supporting the care team in a home-based hospice setting.
Required Qualifications
- RN license in the state you work
- Two years of experience as a registered nurse, hospice experience preferred
- Current driver’s license
- Current CPR and BCLS certification
