Job Summary
Hospice Registered Nurse / RN Case Manager delivers home-based hospice nursing care, builds trusting relationships with patients and families, educates caregivers on patient care, and collaborates with the care team to provide comfort, peace, and dignity for patients at end of life, ensuring high-quality, compassionate care consistent with the Care for Who I Am culture.
Required Qualifications
- RN license in the state you work
- Two years of experience as a registered nurse, hospice experience preferred
- Current driver’s license
- Current CPR and BCLS certification
