Job Summary
Hospice RN Case Manager role delivering home-based hospice care: build trusting relationships with patients and families, educate caregivers on at-home care, collaborate with the care team to provide comfort and dignity, and support patients in spending meaningful time with loved ones. Requires an active RN license in Georgia, at least two years of nursing experience (hospice experience preferred), a valid driver’s license, and current CPR/BCLS certification.
