Hospice Registered Nurse Case Manager

Job Summary
Hospice Registered Nurse / RN Case Manager provides in-home hospice care, conducts patient assessments, develops and implements care plans, and coordinates with an interdisciplinary care team to manage pain and symptoms. Builds trusting relationships with patients and families, educates caregivers on home-based care, and supports end-of-life care decisions to help patients spend meaningful time with loved ones. Requires an active RN license in the working state, at least two years of RN experience (hospice experience preferred), a valid driver’s license, and current CPR/BCLS certification. Works with Compassus’ Care for Who I Am culture to deliver compassionate, high-quality hospice services in the Bloomsburg, PA area.
Required Qualifications
