Job Summary
Hospice Registered Nurse / RN Case Manager provides compassionate, home-based hospice care by building trusting relationships with patients and families, empowering caregivers through education, and collaborating with the care team to deliver comfort and dignity. Responsibilities include enabling patients to spend quality time with loved ones and their activities, educating caregivers on care needs, and working with the team to uphold a high standard of end-of-life care. Required qualifications include an active RN license in the working state, at least two years of RN experience with hospice experience preferred, a current driver’s license, and CPR/BCLS certification.
