Job Summary
Hospice Registered Nurse Case Manager position located in Anchorage, AK. Involves home-based hospice and palliative care with an Interdisciplinary Group to assess patient and family needs, develop and direct individualized plans of care, administer medications and treatments as prescribed, participate in on-call duties, maintain accurate clinical documentation, and mentor staff. Requires completing assessments, writing and initiating Plans of Care, and delivering compassionate, person-centered nursing care in the home setting. Full-time, day shift with potential sign-on bonus; emphasis on community health education, end-of-life care, and collaboration with IDG to achieve goals for patients and families.
Required Qualifications
- Associate's or Bachelor's degree in Nursing from an accredited school
- Current, unencumbered RN license (State of employment)
- BLS - American Heart Association certification (Vendor Managed)
- Minimum 1 year hospice experience preferred
