Job Summary
As a Hospice Registered Nurse / RN Case Manager in Dallas, you will enable patients to spend quality time with loved ones by coordinating home-based hospice care, building trusting relationships with patients and families, educating caregivers on care needs, and collaborating with the care team to provide comfort, peace, and dignity. You will leverage your RN license and hospice experience to assess patient needs, develop and implement care plans, and ensure compassionate, patient-centered support across the care continuum. The role emphasizes end-of-life care, patient advocacy, caregiver education, and coordination of services to support families and caregivers in the home setting.
Required Qualifications
- RN license in the state you work
- Two years of experience as a registered nurse, hospice experience preferred
- Current driver’s license
- Current CPR and BCLS certification
