Hospice Registered Nurse Case Manager

Enable patients to spend quality time with their loved ones by building trusting, compassionate relationships with patients and their families. Educate caregivers on care for their loved ones and collaborate with the care team to provide comfort, peace, and dignity. Maintain an RN license in the state of employment with two years of nursing experience, with hospice experience preferred. Hold a current driver's license and CPR and BCLS certification. This role supports the continuum of integrated home-based services including hospice care, transforming care at home for every community served.
