Job Summary
Registered Nurse Case Manager role in hospice/home health. Responsible for assessing hospice care patients and families to identify physical, psychosocial, and environmental needs; communicating significant findings to the Interdisciplinary Team; counseling and involving patients, facilities, and families in following the Interdisciplinary Plan of Care; teaching hospice philosophy and pain/symptom management to staff; providing bereavement assessment and support at time of death. Requires RN license, at least 1 year of clinical care experience, and willingness to travel up to 50% to patient locations. Travel logistics include having a valid driver’s license and reliable transportation. Required Qualifications
- Current State License as a Registered Nurse (RN)
- 1 year of experience in a clinical care setting
