Hospice Registered Nurse Case Manager

Assess hospice care patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, and interdisciplinary team reports. Counsel, instruct, and include patients, facilities, and families in following the Interdisciplinary Plan of Care while teaching hospice philosophy and pain management to staff in alternate care settings. Communicate significant findings, problems, and changes in health conditions or unsafe facility conditions to the clinical supervisor, physician, and other personnel involved with patient care. Provide appropriate support at time of death and perform bereavement assessments. Ensure all activities align with the vision of Elara Caring's board and leadership. This role requires 50% travel and a current RN license with one year of clinical experience.
