Job Summary
Provide and coordinate hospice nursing care for home-based patients. Deliver hospice nursing care, conduct thorough nursing assessments, plan and implement effective care, manage pain and symptoms, ensure patient safety, educate patients, families, and caregivers on death and dying, and support patients through end-of-life processes. Build therapeutic relationships, document promptly, and uphold Bloom Healthcare standards while collaborating with the interdisciplinary hospice team. Preferred experience in hospice or oncology and CHPN certification.
Required Qualifications
- Accredited nursing school graduate
- Valid CO RN license
- At least one year in med-surg, emergency or critical care nursing
