Job Summary
Hospice RN Case Manager provides and coordinates hospice care for patients in community or home settings; delivers comprehensive nursing assessments, plans and implements care, manages pain and symptoms, educates patients, families, and caregivers on end-of-life processes, ensures patient safety, maintains timely documentation, and upholds Bloom Healthcare standards while collaborating with an interdisciplinary hospice team.
Required Qualifications
- Accredited nursing school graduate
- Valid TX RN license
- At least one year in med-surg, emergency or critical care nursing
- Strong clinical assessment capability
- Excellent communication and interpersonal skills
- Effective problem-solving skills
- Detail-oriented with strong time management
- Computer proficiency
